ELIGIBILITY & APPLICATION PROCESS

Must meet Income Requirements, provide income verification, proof of home ownership, program specific documentation
Owners eligible; mobile homes over 15 yrs old not eligible
Must agree to all services required by State of Ohio & sign work authorization
Must be (60+)

SERVICES PROVIDED

Home repairs are limited to one-time assistance per household.
Not to exceed $3500 per five (5) years.
Modifications for accessibility are not limited to one-time
with the exception of remp installation
not to exceed $5000
Funding Cycle: Jan 1 - Dec 31
